For those unfamiliar: the character creation in BG3 lets you separately pick your character’s pronouns, body shape, and genitals. There’s male/female/enby pronouns, four body shapes (2 masc, 2 femme), and like 8 sets of genitals (uncircumcised penises, circumcised penises, and various vulvae).
It’s no different from other games with character customization except it doesn’t couple gender and physical presentation.
I haven’t gotten far enough into the game to see if genitals make an appearance again, and any clothes you wear seem to override the existence of genitals, so it may have been a marketing thing like Cyberpunk did.

I feel like that’s way too specific for a non-porn game, but like, the other parts to this do clear up some confusion.
